Invitation to Tender for the Portfolio of Surrey Community Health. Introductory Notice. NHS Surrey is required to ensure that the provision of community services in the areas of Surrey not currently served by Central Surrey Health is delivered by an organisation independent of NHS Surrey by 1.4.2011, or as soon as possible thereafter. As of January 2011, NHS Surrey is not seeking to go to the market for the procurement of most services in South East Surrey as there is a bid from staff there for social enterprise status which is progressing to the next stage. That position will be reviewed if the application fails to progress satisfactorily. This decision will be clear by late February. The portfolio therefore comprises services delivered in North West and South West Surrey as well as some delivered Surrey wide, including sexual health services and prison healthcare. The agreed portfolio of the potential social enterprise in the East will impact on the final composition of the portfolio, and potential bidders will be apprised of any changes as they are confirmed. NHS Surrey is looking to transfer the whole portfolio of services "as is", but with the proviso that there will be significant re-engineering of the services over the next twelve months to deliver alternative models of care which provide high quality services at affordable costs. There is an expectation that the new provider will maximise opportunities for integration with social care (for adults and children) and other health providers, such as the ambulance service, voluntary and charitable providers, acute trusts, mental health and learning disability services and primary care. At the same time, an approach to local delivery which respects the particularities of each health economy and the objectives of emerging GP consortia will be required, especially in respect of the day to day work of key community staff. This transfer will take place in the context of public consultation on a new model of care at certain community hospital sites, which is looking to provide urgent ambulatory care in non acute settings, and will require decisions to be made about the number, location and purpose of community hospital services, beds and sites. We expect some sites providing urgent ambulatory care to be destinations of choice for ambulances with triage through NHS Pathways and this development must generate significant savings against current acute spend. Bidders will be asked to describe how this service model might contribute to the process of restoring financial balance to the Surrey system. The contract let will be the standard, national, NHS community contract. In view of the requirement for service re-engineering which will require funding and/or pump priming, the term of the contract will be up to five years; however, there will be the expectation that certain elements of the service will be market tested after eighteen months, under the auspices of GP Commissioning consortia, and emerging policy regarding Any Willing Provider will potentially impact over time. NHS Surrey is looking to progress this procurement process through competitive dialogue, which will centre on the following elements: 1. The effective safe transfer of existing staff and services to a context where terms and conditions will not be a disincentive for recruitment and retention. 2. The development, integration and transformation of services for quality and cost improvement. There is a clear expectation that activity must be shifted from acute to community settings in planned and unplanned care where that is clinically appropriate and less costly, and that this model will deliver substantially on the QIPP agenda. This will see a 20 % reduction in acute hospital admissions by the end of 2013, a 10 % cut in A&E attendance and a 25 % reduction in hospital length of stay by the same time. (Source DH QIPP programme). NHS Surrey will work with potential providers to secure mutual understanding of financial and service delivery models during the competitive dialogue process. The successful bidder is likely to be the one which best assures NHS Surrey, Surrey County Council and potential GP Commissioners of the soundness of its transformational plans in clinical and financial terms, and that it will deliver local accountability to GP Commissioning consortia.
