It aims to deliver a basic service of grass and hedge cutting to all Flintshire residents who qualify for the scheme. The service also aims to integrate a welfare provision incorporating the Council's proposals around transforming social services for adults and building preventative services. More specifically the welfare provision would require an undertaking by suppliers to engage with tenants whilst on site with a view to referring any wellbeing concerns to Council officials for follow up. This may take the form of, knocking on doors, filling out forms and ringing the Neighbourhood Housing Managers in an emergency. It is intend that suppliers accepted on the framework agreement will compete for 'call-off's' throughout the lifetime of the agreement. Community Based Intermediate Care Beds Service for NHS Tameside & Glossop. NHS Tameside & Glossop are inviting expressions of interest to deliver a safe, cost-effective and patient centred intermediate care services within the Tameside locality. In the first instance the cohort of service users are anticipated to be patients being stepped down from acute secondary care. It is anticipated that as ambulatory care pathways are developed locally, the service will provide step up provision from the community to support service users being managed at the most appropriate level of care for their needs. Step up provision is likely to be adopted during the lifetime of the contract and the pace will be agreed with the provider and other stakeholders as patient pathways are developed. There is a requirement to provide up to 40 intermediate care beds in the Tameside locality; commissioners already have access to an inpatient facility at Shire Hill Hospital, Glossop. The Tameside beds will be situated in a suitable location such as a care or nursing home and need to be available for occupancy no later than 1.11.2012, but there maybe the opportunity for an earlier phased implementation. There is a preference at this stage to operate all of these beds from one location; however the commissioner is open to consideration of having the preferred bidder(s) operating from multiple sites within the Tameside locality. We would however be concerned at the impact that more than 2 sites would have on quality of service provision and value for money and the bidder would need to demonstrate very clearly how these requirements would be met in their bid. The procurement will include: — facilities with capacity for up to 40 beds, — nursing and therapy provision, — medical cover NHS. Tameside & Glossop are looking for any preferred bidder to be able to provide the estate plus the nursing/medical cover. General Overview The overall aim of the intermediate care services is to support the rehabilitation and rec. CPV: 85000000, 85100000, 85140000, 85144000, 85144100. ...
