UK-Rhyl: social work and related services
ABBA (Aros Buw Bod Adref) Floating Support Project. NB: this is a voluntary notice in relation to a contract for Part B services. The ABBA (Aros Buw Bod Adref) Floating Support Project is a time-limited, medium intensity floating support project for people who are experiencing early signs of dementia, and who could potentially experience difficulties in maintaining their independence due to memory problems, depression, psychosis or similar problems. Service users will be people of both genders who currently live in Denbighshire and are either homeless or at risk of becoming homelessness. All service users will have been identified as needing support in a number of areas to enable them to independently manage their housing tenure (e.g. their tenancy). It is expected that service users will be supported for a maximum of 12 months (as appropriate to their needs. The project will support a minimum of 20 people at any one time. Support worker hours will be managed dynamically and will be allocated flexibly in response to service user need. Service user numbers will normally be expected to exceed the minimum level as new service users join the project while support is gradually withdrawn from those service users approaching the completion of their support. The project will be expected to have the capacity to accept urgent referrals as may be required from time to time. Staff will deliver support activities in service users' own accommodation and the community. Project Aims: — enabling people with early stage dementia and housing related support needs to live as independently as possible in the community, — avoiding dependence and delaying the need for Community Care Services, — supporting service users to access assistive technology (NB: this project will not act as a response service), — contributing to minimising risks associated with unsafe behaviour such as wandering, — enabling service users to access advice and support regarding home safety, — supporting people to establish and maintain positive relationships with family, friends, neighbours and their communities, — providing support to avoid financial crises including rent/mortgage arrears etc, — supporting service users to access advice from Denbighshire County Council's Receivership Officer (NB: Service users will not be eligible for support if financial control is retained by the Receivership Officer), — assisting service users to access longer term or higher level support/care when or if this may become necessary, — (NB this service should not be used to discharge a statutory social service duty). Project Outcomes: — service users will be better able to access and engage with the services they need, — service users will be better able to manage their lives and affairs, — service users will be better able to avoid or cope with crises and other situations which cause stress or anxiety. NB: the project will also contribute to achieving the following outcomes and actions detailed in Denbighshire's Big Plan: — older people lead independent and fulfilled lives, — the needs of Denbighshire's rural communities are recognised and met, — vulnerable adults in Denbighshire are safe, — support early diagnosis and support for people with dementia to prevent the need for long-term intensive support. Referrals: Referrals will be accepted from Denbighshire County Council Social Work teams, the Elderly Mental Health and Community Mental Health teams and other statutory and voluntary agencies including the Alzheimer's Society via the Denbighshire County Council Supporting People Coordinator. Refused referrals or those placed on a waiting list will all be promptly referred to the Supporting People Coordinator for possible re-allocation to another project. Any voids or vacancies on the project will be promptly notified to the Supporting People Coordinator and other referrers. Service users will be selected on needs alone and not on their prospects of successful move on. NB: if an ongoing support need is established (i.e. a long term requirement for support extending beyond 2 years), the service users will be referred to a more appropriate low level or non-SP funded service in accordance with Supporting People eligibility criteria.
