Expressions of Interest to Deliver 24 Hour Residential Service @ 13/14 High Mount Donnington Telford TF2 7NL Telford & Wrekin Council is seeking expressions of interest from potential providers who are interested in delivering a 24 hour Residential Care Service provision. Expressions of Interest are invited from suitably experienced and qualified care providers, who wish to be considered in providing this service. The Service The intended service provision is to offer 8 adults (some of whom are older), with severe learning disability complex needs and behaviour that may challenge a residential based service with personal care and support and increased level of day care provision. The successful provider must deliver the service that demonstrates service users' rights and responsibilities; to have their voices heard that will restore their personal autonomy and social integration leading to an independent, rewarding and valued life. The successful Provider will be expected to provide and deliver the service in manner which reflects the Councils overall commitment to Personalisation and Enablement. The Provider will be expected to facilitate and implement stimulating opportunities /activities that are person centred and promote social inclusion within the community. Premises and Lease 13 and14 High Mount is two purpose-built bungalows and has been in operation for over twenty years. The properties are situated in the north boundary of the borough. Both properties comprise of 4 single bedrooms. There is a sleep in room in situated in number 13. Collaboration during Transition The Council owns the premises from which the service will continue operate. The new Provider is required to enter into a direct full repairing lease with Telford & Wrekin Council; the full details of the lease will be included in the invitation to tender pack. The Council is seeking to conclude a new contract with a possible implementation period, with the service provision to commence from 31.5.2012 The exiting providers have indicated their wiliness to work alongside the council, service users, family, carers and staff to ensure continuity of service provision. Any incoming provider would likewise be expected to work with the council to ensure a seamless handover which does not adversely affect service provision, service user or staff members. Providers who are interested will be required to submit their tender and will be subject to the Council's assessment and evaluation process.
