Service Specification: To provide Hounslow's Residents with the opportunity to access modernised community resources that move away from a traditional mental health day centre model and that are more closely aligned with the flexible and person centred community resources described in Government policy and commissioning guidance. The Community Mental Health Resources Service will be a transformational service that enables Residents to avoid and in particular, to reduce the need for, prolonged hospital admissions and dependency on mental health services.
