Provision of Community Health Trainer Services. NHS Gloucestershire wishes to commission a provider to manage and develop the existing Gloucestershire Health Trainer Programme, consistent with national guidelines and core competencies. NHS Gloucestershire has developed and managed the programme since September 2008, steered by a multi-agency Reference Group. The aim of the programme is to support adults in Gloucestershire from targeted geographical areas (20 % most deprived) and communities of interest (e.g. offenders) to achieve and maintain behaviour change to improve their health and wellbeing. The programme supports the Healthy Gloucestershire ambition to reduce health inequalities. The service will include: 1. Management, supervision and professional development of the existing team (Co-ordinator, Project Officer and 6 Community Health Trainers); 2. Recruitment of additional Community Health Trainers and volunteer Health Champions; 3. Managing contracts for the delivery of accredited training – Level 3 City and Guilds for Health Trainers and Level 2 RSPH Understanding Health Improvement for Health Champions; 4. Delivery of targeted one-to-one life-style advice and signposting for adults in identified deprived areas and communities by the Community Health Trainers; 5. Delivery of community outreach and dissemination of health improvement messages by the Health Champions; 6. Maintaining and developing relationships and referral networks with statutory, voluntary, community and private sectors; 7. Accurate data collection, monitoring and evaluation to inform the local and national evidence for staying healthy and reducing health inequalities. The last lot with a corridor length of approximately 600 km is part of this call for competition and will be based on the available orthophotographs, LIDAR data (Las format) and a DTM (point x, y, z).
